چکیده

A 20-year-old man presented with vertigo followed by hourly episodes of dysarthria and incoordination lasting 5–20 seconds (video 1). Neurologic examination was normal between episodes. The only brain/spine MRI lesion in the right superior cerebellar peduncle (figure, A). CSF analysis revealed elevated white blood cells (6/?L [normal, 0–5]) oligoclonal bands (?4). Paroxysmal dysarthria–ataxia associated CNS demyelination diagnosed resolved oral acetazolamide 250 mg twice daily. Subsequent brain spine B) confirmed multiple sclerosis. arises from ephaptic transmission within midbrain or sclerosis lesions may respond to carbamazepine acetazolamide.1 It should be distinguished genetic contactin-associated protein-like-2 (CASPR2) immunoglobulin G–associated episodic ataxia.2

Paroxysmal dysarthria and other transient neurological disturbances in disseminated sclerosis.

The alternation of relapse and remission is one of the outstanding characteristics of disseminated sclerosis. Relapse has been defined as 'the appearance of a new symptom . . . or the return or exacerbation of a previous symptom' (McAlpine, Compston, and Lumsden, 1955). Classically, these symptoms last for days, weeks, or months. Distinct from the true relapse, the 'fluctuation' (Muller, 1949),...

Teaching video neuroimages: complicated scapular winging.

Scapular winging (SW) is a common sign in neuromuscular disorders. Besides “pure” phenotypes due to single muscle weakness often secondary to nerve injuries or dysfunctions, the phenotype can be complicated when a combination of different scapular fixators is involved by a myopathy. We show an example of 2 sisters with facioscapulohumeral muscular dystrophy (video 1 on the Neurology® Web site a...
